Why Running Late Is a Clinical Problem, Not Just an Annoyance
Every patient has sat in a waiting room forty minutes past their appointment time. It is irritating, but the real cost is upstream of the waiting room. A practice running that far behind is a practice where each appointment was booked shorter than the work actually takes, and the pressure of that lands on the treatment.
Dentistry is a manual craft performed in a small, wet, moving space. The difference between a filling that lasts fifteen years and one that fails in three is very often whether the tooth was kept properly dry while the material set, whether the margin was finished carefully, and whether the bite was adjusted to within a fraction of a millimetre afterwards. Every one of those steps is the kind of thing that gets shortened when the next patient is already in the waiting room.
So booking realistically is not a hospitality decision. We schedule appointments for the time the work takes, which means we see fewer people in a day than a high-volume clinic and we start on time. If you need longer — because you are nervous, because you have mobility issues, because you want to ask questions and not feel hurried — you ask, and we book it. There is no charge for the extra time.
The same principle applies to how much gets attempted in one sitting. There is a style of dentistry that fits four fillings into one long appointment because it is efficient for the practice. It is often not better for the patient, particularly an anxious one. Splitting treatment into shorter, calmer visits is almost always available if you would prefer it.

Questions About Nervous and Unhurried Care
I have avoided the dentist for years because of anxiety. Where do I even start?
Start with a phone call rather than an online booking, and say the word anxious. That single sentence changes what gets booked: a longer appointment, at a quieter time, with the understanding that this first visit may be nothing more than sitting in the chair and talking. Some patients have an examination on that first visit and some do not, and both are fine. What matters is that you leave having had an experience that did not confirm your fears, because the next appointment then becomes possible.
Does numbing always mean a needle, and does the needle have to hurt?
For anything involving the drill, yes, you need local anaesthetic, and yes it is delivered by injection. What is negotiable is how it feels. Topical numbing gel on the gum for a full minute beforehand, warmed anaesthetic solution, and delivering it slowly rather than quickly make a very large difference — most of the discomfort people remember from injections comes from the solution being pushed in fast, not from the needle going in. Ask for it to be done slowly. Any dentist who is not willing to spend the extra ninety seconds is telling you something.
What is a hand signal and does it actually work?
You raise your left hand and everything stops — drill off, instruments out, no questions asked. It works because it gives you control, and loss of control is what most dental fear is actually about. Lying flat with someone's hands in your mouth is inherently a position of helplessness, and knowing you can end that at any moment changes how the whole appointment feels. We agree it before we start and we honour it every single time, including when you use it three times in ten minutes.
I have a very strong gag reflex. Is there anything that can be done?
Quite a lot, actually. Intraoral scanning has replaced impression putty for most purposes, which removes the worst trigger. Digital x-ray sensors are smaller than old film, and some views can be taken from outside the mouth entirely. Sitting more upright rather than reclined helps, as does breathing steadily through the nose and keeping your feet crossed or your legs raised, which sounds absurd and genuinely works for some people. A pinch of salt on the tip of the tongue is an old trick that has decent evidence behind it. Tell us and we will work around it.
Can I bring my partner into the room with me?
Yes. A support person in the surgery is welcome and it makes a real difference for a lot of patients. The only thing we would ask is that they let you answer questions we ask you, because we need to hear from the person in the chair.
Do you use sedation or laughing gas?
Our approach for the great majority of anxious patients is time, technique and control rather than sedation, and it works for most people who thought it would not. Where anxiety is genuinely severe enough that treatment is impossible awake, the right answer is sedation or general anaesthetic delivered in a properly equipped setting with appropriate monitoring, and we will refer you for that rather than improvise. What we would suggest is coming in and talking first, because a significant number of people who arrive convinced they need to be unconscious end up not needing it.
Will you actually run on time?
That is the intention and it is why we book fewer patients per day than we could. Emergencies do occasionally disrupt a morning, because someone in genuine pain gets seen, and on those days we will tell you at the desk rather than leaving you guessing. If you are ever kept waiting without an explanation, say something — it means something has gone wrong with our scheduling and we would rather know.
How long is a standard appointment?
A check-up and clean is typically scheduled around 45 minutes, longer for a first visit with x-rays and a full history. Fillings vary with size and number. If you want more time than the standard slot for any reason, ask when booking — it costs nothing and it is far easier to allocate in advance than to find on the day.
